MECHANICS:
1. Present your valid Vaccine Card (fully vaccinated) at any of our participating establishments (local restaurants, pharmacies, grocery stores, hospitals, wholesalers, retailers, etc.).
2. Whenever you make a minimum purchase of ₱350.00 (any item or service) at a participating establishment, a raffle ticket will be given to you for FREE.
3. Fill in the ticket stub with the ff information:

(Note: Don't bother presenting fake vaccine cards, winners will be validated with the CHO master list of vaccinated individuals, so if you're not on the official list, your prize will be forfeited automatically.)
4. Attach the original Official Receipt to the ticket stub, and drop in the Drop Box available at the participating establishment's premises.
5. Winners will only win ONE TIME. Once your ticket is drawn, all other subsequent ticket stubs with the same name will be forfeited in favor of the first ticket drawn.
6. Grand Raffles will be held on January 30, 2022.
7. To be a PARTICIPATING ESTABLISHMENT, please submit a complete list of all your employees (to LEDIPO) , showing their vaccination status, and attach photocopies of their respectIve Vaccination Cards. Only establishments with 100% FULLY VACCINATED employees may join.
8. Promo is open only to residents of Isabela City with Vaccination Cards issued only by the Isabela City Health Office.
9. The following establishments have been confirmed by CHO to have 100% fully vaccinated employees and are therefore eligible to participate in the promo:
