Q: Immunity (or lack thereof) is the bit I find confusing. Itโ€™s obviously not killing most who contact the diseaseโ€”so there must be some ability to fight this thing in healthy folks. Is it then a matter of viral load, or less virulent/lethal strain of the thing?

A: Immunity occurs after our bodies have seen a virus before and then 'learns' how to defend against the offending organism. Our white blood cells become sensitized to the virus and can recognize it when it sees it again, and attack it appropriately.

With coronavirus, our bodies are seeing this virus for the first time so we don't have any immunity. In some people, their immune systems totally overreact. It's these overreactions that are suspected of being the cause of the high fatality rate especially in otherwise healthy adults.

When the immune system overreacts, there is something called a 'cytokine storm' that happens. It's like the immune system goes to nuclear, and unleashes all of the white blood cells at once to go to war and they bomb everything they have, at everything in sight - in essence, attacking the virus but also the body and different organ systems at once. (It's similar in concept to when people die of allergic reactions).

This may also be why children are less affected since their immune systems are not as mature.

Meanwhile, elderly people or those with underlying disease are at higher risk of severe disease and death bc their bodies are also not as able to defend themselves in the first place, and they can't fight off the virus.

As to why some people are affected seriously and others are not - we still don't totally understand, which is why preventing/slowing spread is the best strategy that we have. -MP

A PRIMER ON COVID-19 - Pt. 6 (last one)

We need to 'flatten the curve' to preserve health systems capacity.

Health systems capacity sounds like a vague term but what it means in concrete terms is: we want to make sure we have enough hospital beds and equipment (primarily ventilators, but also things like dialysis units and ECMO machines) so we can treat the patients that have a severe case of COVID-19 that need them in order to stay alive and recover. Capacity also includes everyone who works in healthcare. Again, we want to prevent the spread of infection to protect our front-line healthcare workers as much as possible, so they can stay healthy and do the vital work of keeping our loves ones alive.

Remember - our hospitals are already treating patients that need a high level of care (our ventilators aren't just sitting there waiting to be used - we're already using most of them!). We also only have so many ventilators, hospital beds, and doctors and nurses.

For the sake of example, let's say that 20% of people in Chase are infected at one time. That is roughly 500 people. Let's assume that 10% of them will require hospitalization (50 people), and of those, 10% will need mechanical ventilation support (5 people). Assume that everyone needing ventilation needs to stay in hospital for at least 10 days.

For hospitals in the Chase area, we're talking about Royal Inland Hospital in Kamloops and Shuswap Lake General Hospital in Salmon Arm.

To continue with this example: from Chase, 50 patients come in to Royal Inland needing to be hospitalized, and of those, 5 need ventilation. This is already almost one-quarter of the hospital's total ~225 beds.

The same thing is happening in Kamloops and the surrounding communities that the hospital serves. Ventilators and hospital beds are pushed to full capacity. Meanwhile, hospitals in the lower mainland are filling up with patients from Vancouver so any new incoming patients can't be sent there. With each passing day, more people get infected and need to be hospitalized... leading to the impossibly tragic situation where our health system does not have the capacity to treat the influx of COVID-19 patients. We will be overrun, and as a result, people will die.

Hope this paints the picture of what could happen.

The thing is, it does NOT have to happen this way. Let's say we properly distance and isolate and are successful in slowing down the spread of COVID-19. Then, we wouldn't have an influx of cases coming to the hospital all at once and creating a bottleneck.

We can slow & prevent the spread of infection if we distance properly, and take all the necessary precautions that we can. This is why it is important for ALL of us to stay home, and distance as much as possible. Each individual action is going to add up and make the difference for everyone.

More on flattening the curve: https://www.nytimes.com/2020/03/11/science/coronavirus-curve-mitigation-infection.html

One chart explains why slowing the spread of the infection is nearly as important as stopping it.

A PRIMER ON COVID-19 - Pt. 5 (of many)

I wish that were the case, but it's not. We already have widespread transmission all over Canada and also in Interior BC. There are documented cases very close to and around Chase, from weeks ago. I do not know why the government is not releasing the location information of cases at this time.

We also have what is called 'community transmission' in BC, meaning that there are people who have COVID with no documented travel history and didn't get it from elsewhere. This means they got it from someone else in the community, and that there are infected people that we don't know about - that have probably already infected others, and so on. All of this means that there is already enough of the virus circulating in the population for us to be concerned.

Right now, as a society, we are trying to 'socially distance' as much as possible so we can slow down and prevent the spread of infections ('flatten the curve'). This allows us to preserve our health systems' capacity for as long as possible (more on that in the next post).

We also know that elderly people and those with existing health conditions are at higher risk of having a severe case of disease. And this fits the demographic description of Chase and similar communities.
