In the case of a lost receipt for your pre-ordered items, such as the NSTP shirt and P.E. uniform, there is NO LONGER a need for an affidavit of loss.

For a hassle-free process, kindly proceed to the COOP Office to request a Certified Copy of your receipt, with a nominal fee of 10 pesos.

Please be advised that the release of pre-ordered items will only occur upon presenting either the original receipt or its certified copy.

This also applies to anyone seeking a Certified Copy of their receipt for items bought at COOP
