Does your NICU use neonatal delirium as a diagnosis? Currently, this topic is of high interest, especially in Level IV NICU settings. Delirium can be affiliated with lower neurodevelopmental outcomes and is preventable.

As neonatal therapists, we want to know more!

• 🤔 What does delirium look like in the neonatal intensive care?
• 📚 What are the recommended interventions found in the literature?
• 🌐 Where can I learn more?

Let’s begin with the definition according to the Diagnostic and Statistical Manual of Mental Disorders-5:
“Neonatal delirium is defined as the acute onset of disturbances in attention, awareness, cognition, and consciousness resulting from an underlying medical condition.”

Delirium is especially difficult to assess in the NICU population because the characteristics are less defined in neonates with earlier neurodevelopmental ages and stages. In general, delirium is categorized in three subtypes: hypoactive, hyperactive, and mixed. Unfortunately, there is a paucity of evidence related to the diagnosis and treatment guidelines for neonatal delirium. Current screening tools target older pediatric patients (i.e., the Cornell Assessment of Pediatric Delirium) and are not designed for preterm and medically complex newborns.

What can we do to help? 👀 Watch for signs of agitation, motor restlessness, disrupted sleep-wake schedules, poor responsiveness to escalating need for sedatives, and arousal concerns, and report them to your team.

Therapists are also helpful in promoting non-pharmacological interventions to support regulation such as environmental modifications, positioning, safeguarding sleep, and promoting family involvement. 🛏️👶
